The Best Same Day Doctor in Vista, CA
Coastal Internal Medicine offers same-day doctor appointments in Vista, California, for adults experiencing acute illness, infections, flu symptoms, or sudden health concerns. Led by Dr. Mark O'Brien, D.O., a board-certified internal medicine physician with over 30 years of experience, our practice provides personal, continuous care — without the long waits and rotating providers of urgent care clinics. Call (760) 472-5195 as early in the day as possible to reserve a same-day slot. We serve patients across Vista, Oceanside, Carlsbad, San Marcos, and Escondido. For medical emergencies, call 911.
When Same-Day Appointments Make Sense
Some health concerns can't wait days or weeks for a routine appointment, but also aren't emergencies that require the ER. Same-day appointments at Coastal Internal Medicine are designed for situations like:
Cold, flu, and COVID-19 symptoms — fever, body aches, severe cough, congestion
Respiratory infections — bronchitis, sinus infections, persistent coughs
Urinary tract infections (UTIs) — burning, frequency, urgency
Skin infections, rashes, and unexplained skin changes
Sore throat and strep symptoms
Pink eye and other eye irritations
Mild allergic reactions
Sudden flare-ups of chronic conditions — like elevated blood sugar in diabetic patients, blood pressure spikes, or worsening asthma
New, concerning symptoms that need a physician's evaluation but aren't life-threatening
If you're not sure whether your situation needs a same-day visit, call our front office at (760) 472-5195. Our team will help you decide whether to come in today, schedule a regular visit, or seek emergency care.
Why See Your Own Doctor Instead of Urgent Care
Urgent care clinics serve a purpose — but they're built for one-time visits with a provider who doesn't know your history, your medications, or your chronic conditions. That can lead to over-testing, redundant prescriptions, and care that doesn't connect to your bigger health picture.
Same-day visits at Coastal Internal Medicine offer something different:

You see Dr. O'Brien
the same physician who manages your overall health, not a rotating cast of providers

Your full medical history is in the room
medications, allergies, chronic conditions, recent labs, prior diagnoses

Care continuity
What happens today is documented in your chart and informs every future visit

No upselling of unnecessary tests or imaging
Typically lower out-of-pocket cost than urgent care

Chronic Illness FAQs
Do you offer walk-in appointments?
We don't offer formal walk-ins, but we do offer same-day appointments by phone. Call (760) 472-5195 as early in the day as possible and we'll work to fit you in.
What time should I call for a same-day appointment?
The earlier the better. We open at 9:00 AM Monday through Friday, and same-day slots fill quickly — calling at opening gives you the best chance of being seen that day.
Will my insurance cover a same-day visit?
In most cases, yes — same-day visits are billed the same as regular office visits. Coastal Internal Medicine accepts Medicare, Aetna, Anthem Blue Cross, Blue Shield, Cigna, United Healthcare, and Greater Tri-Cities IPA. Out-of-pocket costs are typically much lower than urgent care or ER visits.
What if I just need a prescription refill or a quick question answered?
You don't always need an appointment for these. Call the office or use our patient portal — for established patients, refills and brief clinical questions can often be handled without a visit.
Can Dr. O'Brien treat my child for a same-day issue?
Coastal Internal Medicine is an internal medicine practice, which means we treat adults age 18 and older. For pediatric care, we recommend a family medicine or pediatric provider.
What if I have a chronic condition that's flaring up?
This is exactly what same-day visits are designed for. If your diabetes, blood pressure, asthma, or another chronic condition is acting up, call (760) 472-5195 — Dr. O'Brien already knows your history and can adjust your treatment plan the same day.
