What Is a Primary Care Physician?
A primary care physician (PCP) is the doctor you see first for most health concerns — the central point of contact for your medical care. A good primary care physician knows your full health history, manages your medications, coordinates specialist referrals, performs your annual physical, treats common illnesses, and helps you stay healthy through preventive care.
At Coastal Internal Medicine, Dr. Mark O'Brien serves as the primary care physician for adults across North San Diego County. As a board-certified internist, he specializes in the prevention, diagnosis, and treatment of conditions affecting adults — from routine checkups to complex chronic disease management.
The relationship between you and your primary care physician is one of the most important in your healthcare. It's a long-term partnership built on trust, continuity, and a doctor who actually knows you — not a rotating cast of providers you meet for the first time at every visit.
What Internal Medicine Means
Internal medicine is the medical specialty focused exclusively on adult healthcare. Physicians like Dr. O'Brien — known as internists — complete four years of medical school followed by three or more years of residency training devoted entirely to adult medicine. This deeper, narrower training gives internists particular expertise in:
Complex chronic disease management
Patients with multiple coexisting conditions
Adult preventive care and screening
Diagnosis of difficult or undefined symptoms
Coordination of specialist care for complicated cases
If you're an adult — especially one managing one or more chronic conditions — an internist is often the best fit for primary care. Internists are sometimes called "the doctor's doctor" because of how often other physicians refer their own families to them.

A few things make Dr. O'Brien's practice different:

Doctor-led, not corporate
Coastal Internal Medicine is independent — not part of a hospital system or private equity rollup. Decisions about your care are made by your doctor, not by an administrator.

Time with patients
Visits are scheduled to allow real conversation, not the 8-minute hurry typical of large practices.

Long-term continuity
Many of Dr. O'Brien's patients have been with him for years because they love the service

Hospital experience
Three decades overseeing inpatient care means Dr. O'Brien has seen virtually every clinical scenario — and brings that judgment to outpatient practice.

Compassion and rigor
Patients consistently describe him as thorough, attentive, and genuinely caring. Colleagues describe him as the "gold standard" of internal medicine.

Chronic Illness FAQs
Do you offer walk-in appointments?
We don't offer formal walk-ins, but we do offer same-day appointments by phone. Call (760) 472-5195 as early in the day as possible and we'll work to fit you in.
What time should I call for a same-day appointment?
The earlier the better. We open at 9:00 AM Monday through Friday, and same-day slots fill quickly — calling at opening gives you the best chance of being seen that day.
Will my insurance cover a same-day visit?
In most cases, yes — same-day visits are billed the same as regular office visits. Coastal Internal Medicine accepts Medicare, Aetna, Anthem Blue Cross, Blue Shield, Cigna, United Healthcare, and Greater Tri-Cities IPA. Out-of-pocket costs are typically much lower than urgent care or ER visits.
What if I just need a prescription refill or a quick question answered?
You don't always need an appointment for these. Call the office or use our patient portal — for established patients, refills and brief clinical questions can often be handled without a visit.
Can Dr. O'Brien treat my child for a same-day issue?
Coastal Internal Medicine is an internal medicine practice, which means we treat adults age 18 and older. For pediatric care, we recommend a family medicine or pediatric provider.
What if I have a chronic condition that's flaring up?
This is exactly what same-day visits are designed for. If your diabetes, blood pressure, asthma, or another chronic condition is acting up, call (760) 472-5195 — Dr. O'Brien already knows your history and can adjust your treatment plan the same day.
