What Is Health Optimization?

Health optimization is preventive, proactive medicine — the practice of identifying risks before they become problems, supporting long-term wellness, and helping you function at your best rather than waiting for something to go wrong.

For most of medical history, doctors have been trained to treat disease. Health optimization flips that approach: instead of asking "what's broken?", we ask "what's possible?" That means rigorous preventive screening, evidence-based lifestyle medicine, and selective use of modern tools — like advanced lab testing, hormone optimization, and weight management medications — when they meaningfully improve outcomes.

At Coastal Internal Medicine, health optimization is grounded in the same clinical rigor Dr. O'Brien has practiced for over 30 years. We don't push unproven therapies or expensive packages of unnecessary tests. We focus on what the evidence supports and what genuinely helps you live longer, healthier, and stronger.

Who Health Optimization Is For

Health optimization at Coastal Internal Medicine is for adults who want to take an active role in their long-term health. This includes:

  • Adults in their 40s, 50s, and 60s who want to catch problems early and stay ahead of age-related decline

  • High-performing professionals who can't afford to be derailed by preventable health issues

  • Patients with strong family histories of heart disease, cancer, diabetes, or other conditions who want rigorous early screening

  • People recovering from a health scare who want to make sure it doesn't happen again

  • Adults who feel "off" but have been told their labs are normal — sometimes the right testing tells a different story

  • Patients interested in hormone optimization or medical weight management who want a board-certified internist guiding the process

  • Anyone tired of rushed appointments that don't address the bigger picture of their health

You don't need a specific condition to benefit from health optimization. You just need to care about how you'll feel and function in 10, 20, and 30 years.

Annual Physicals and Wellness Exams

A comprehensive yearly check-in covering physical examination, vital signs, blood work, age-appropriate cancer and cardiovascular screenings, vaccinations, mental health screening, and lifestyle review. For most adults, the annual physical is covered at 100% by insurance and remains one of the highest-value uses of healthcare.

Executive Physicals

An extended, premium version of the annual physical for adults who want a deeper dive into their health. Executive physicals typically include extended consultation time, advanced lab work, cardiovascular risk assessment, cancer screening review, body composition analysis, and a comprehensive written summary with personalized recommendations. Ideal for high-performing professionals, frequent travelers, and anyone who wants more than a 20-minute checkup.

Preventive Screenings

Evidence-based screening for the conditions most likely to affect adult health, including:

  • Cardiovascular — blood pressure, lipid panels, advanced cholesterol testing when indicated, EKG, cardiovascular risk scoring

  • Cancer — colorectal screening referrals, lung cancer screening for eligible patients, skin checks, age-appropriate breast and prostate screening

  • Metabolic — diabetes screening, A1C, fasting glucose, insulin resistance evaluation

  • Bone health — DEXA scan referrals for osteoporosis screening

  • Other — thyroid function, kidney function, liver function, vitamin D, B12, and other targeted testing when clinically appropriate

We don't believe in screening for the sake of screening. We test what's worth testing based on your age, risk factors, and goals.

Vaccinations and Immunizations

Up-to-date immunizations for adults, including annual flu vaccines, COVID-19 boosters, RSV vaccines for eligible patients, shingles (Shingrix), pneumonia (Prevnar, Pneumovax), Tdap, hepatitis, and travel-related vaccinations when needed.

Hormone Testing and Optimization

Comprehensive hormone evaluation for adults experiencing symptoms like fatigue, low libido, weight gain, mood changes, sleep issues, or loss of muscle mass. Testing covers thyroid function, testosterone (total and free), estrogen and progesterone where relevant, cortisol, and other endocrine markers. When optimization is medically appropriate, Dr. O'Brien develops a personalized treatment plan — including coordination with endocrinology specialists when needed. We approach hormone therapy with clinical rigor, not as a one-size-fits-all sales pitch.

Weight Management (Including GLP-1 Medications)

Evidence-based weight management for adults living with obesity or weight-related health conditions. Our approach includes metabolic evaluation, nutritional counseling, exercise guidance, behavioral support, and medication management when clinically indicated — including newer GLP-1 medications (semaglutide, tirzepatide, and similar) when appropriate for your specific health profile and covered by your insurance. We help you understand the options, the tradeoffs, and what will actually work for you long-term.

Nutrition and Lifestyle Counseling

Personalized guidance on eating, exercise, sleep, stress management, and other lifestyle factors that drive long-term health. Dr. O'Brien is familiar with a range of evidence-supported approaches — including Mediterranean, low-carb, ketogenic, and carnivore-style eating patterns — and works with you to find what fits your goals, biology, and lifestyle. We coordinate with registered dietitians for patients who want deeper nutritional support.

Sleep and Fatigue Evaluation

Persistent fatigue, poor sleep, snoring, daytime drowsiness, and brain fog often have identifiable, treatable causes — sleep apnea, hormonal imbalances, nutritional deficiencies, mood disorders, or medication side effects. We evaluate the full clinical picture, order sleep studies and lab work when indicated, and coordinate with sleep medicine specialists for complex cases.

Men's and Women's Health

Sex-specific preventive care including testosterone evaluation and optimization for men, perimenopause and menopause management for women, sexual health concerns, urinary health, and age-appropriate screenings. We discuss what most rushed primary care visits skip — comfortably, honestly, and without judgment.

What to Expect as a New Patient

Health optimization at Coastal Internal Medicine is a relationship, not a transaction. Most patients follow this general path:

Initial consultation

A comprehensive intake covering your medical history, current medications, family history, lifestyle, goals, and concerns. Plan for an extended first visit.

Baseline testing

Lab work and other testing based on your specific situation. We test what matters; we don't pad the bill.

Personalized plan

A written summary of findings, recommendations, and next steps — built around your goals, not a template.

Active treatment phase

If we're addressing specific issues (hormone optimization, weight management, sleep, etc.), this phase involves more frequent visits and adjustments.

Ongoing maintenance

Once you're stable and optimized, most patients return every 3 to 12 months for monitoring, adjustments, and continued preventive care.
